CONFIDENTIALITY & CONSENT
All information concerning the participants at the Company is kept confidential. Access to this information is available to the participant or their representative, the community music facilitator or the Counsellor and  or company senior staff. I also understand that Special Trained Employees in  Salsa Music Therapy ™  may be involved with music therapy sessions as part of their therapies assessment and that volunteers and work experience students may also be present. Also Family members, social workers and others  employees such  as cares of a disable participant would be able to stay in the session. Salsa Music Therapy™

 Students involved in the sessions are allow to participate in the session.

All the participant of salsa Music Therapy ™ will be Informed  and requested a consent is a legal procedure to ensure that a , patient, client, and research participants are aware of all the potential risks and costs involved in a treatment . The elements of informed consent include informing the client of the nature of the treatment, possible alternative treatments, and the potential risks and benefits of the treatment.

In order for informed consent to be considered valid, the client must be competent and the consent should be given voluntarily.

FEES
 Our fees are by Groups and Individual or Corporate. Fees are invoiced on a term basis at the beginning of each  session. These fees are non-refundable (Payment can be made by cash, cheque or electronic transfer, payable within 14 days of the invoice date.

RESPONSIBILITY OF PARENTS & CARERS
 Because it’s a scheduled session, is   expected that parents and/or carers will:

 Ensure that the participant arrives in time for the session.

Inform  to Our Office  about significant changes in the participant’s life circumstances such as medication, change in behaviour, bereavements, family circumstances, school,  or any other circumstances that would be affecting the normal attendance.

 ATTENDING and  Be present in the program room during the sessions at all times.

VIDEO RECORDING & PHOTOGRAPH OF SESSIONS
We would like to have the right to keep , recorded and take pictures of each session in daily basic. All the material could be used in Video tapes, newsletter, or in our website as a material of testimonial and  photos also can be used as promotional material. If you are disagree with this procedure please notify us about your desire not participate.

RIGHT TO REFUSE SERVICE
The program facilitator has the right to refuse service to participants.

CUSTOMER SERVICES OR COMPLAINTS
 Your  feedback is important to us.

 If you have a question or complaint regarding the functioning of STAFF OR ANY services these will be considered carefully and responded to as promptly as possible. Always report it by writing.

INCIDENTS
Our Company  has an Incident Report on site. All injuries are to be reported and recorded in the Register of Injuries.

I have read and understood  SALSA MUSIC TRERAPY  ™ terms and conditions and any questions I have asked have been answered to my satisfaction.
